**Case Manager**
**Pay scale: $38.10 - $40.85 per hour + shift penalties**
**About us**:
At Sommerset Community Services, we deliver one-on-one in-home support and community participation services for people living with a disability in the Ipswich and Somerset regions. This role will be based at Kingaroy and connect in with our facility in Fernvale. We also offer support for older people living independently in the community who would like to enjoy activities in a social situation.
**About the role**:
As a Case Manager you will work directly with individuals living with a disability and/or mental health condition helping them to increase their independence, maintain their wellness and ultimately improve the quality of their life.
**Responsibilities will include**:
- Provide case management to individuals living with a disability and/or mental illness in keeping with the guidelines contained in the service agreement and plan.
- Utilise Evidence Based Practice in the development and implementation of client's individual support plans.
- Have a willingness to learn on the job.
- One-on-one client facing work including medication assistance and basic personal care.
- Complete, maintain and store documentation in a confidential manner.

This position requires maintenance of a positive Working with Children (Blue Card), NDIS Worker Screening Clearance and Current Australian Driver's License.
