About the Role We are seeking a skilled and compassionate Case Manager to join our Family Based Care team. The successful candidate will work collaboratively with children, young people, their families and carers to develop and implement case plans that aim for permanency goals. This role involves building strong functional relationships with all parties involved, ensuring every voice is heard and every need is met. Key Responsibilities: * To collaborate closely with children, young people, their families and carers to create effective care plans that strive towards achieving permanency goals; * To establish strong functional relationships with both the children and their families; Requirements: 1. A relevant bachelors degree such as Social Work or similar; Requisite qualifications may be desirable but not essential for Aboriginal applicants who possess relevant experience in this field; Relevant bachelors degree from other countries may be accepted if accredited by Australia's relevant government authority;
