Associate Nurse Unit Manager - Discharge Planner
Flinders and Upper North Local Health Network – Port Augusta.
Base Salary Range: $107,487 - $114,917 per annum (pro rata). Part‐time 0.74 FTE.

About the Role
You will provide person-centred nursing care and case management while supporting the coordination of safe and effective patient discharge planning. Working closely with the Nurse Unit Manager and multidisciplinary teams, you will assist with service coordination, patient flow, practice improvement and day‐to‐day operational activities to support positive patient outcomes. You will also provide leadership, guidance and clinical support to less experienced staff while contributing to education, quality improvement and continuity of care across the service.
Key Responsibilities

About You
* Registered or eligible for registration with the Nursing and Midwifery Board of Australia with a current practising certificate.
* Minimum 3 years post‐registration experience or currently classified as a Clinical Nurse/Midwife.
* Strong communication, problem‐solving and organisational skills with the ability to work effectively within multidisciplinary teams.
* Demonstrated competence in nursing practice and experience supporting students and less experienced staff.
