You'll lead with purpose as a Care Manager, supporting your team and making a meaningful difference to the lives of our residents every day. Your role will involve managing and leading the delivery of person-centred care for residents, ensuring the highest standard of ongoing assessment, care planning and evaluation.
Some key aspects of your responsibilities include:
* Overseeing daily clinical operations to ensure residents receive the best possible care
* Supporting and leading the care home's clinical team, including coaching, mentoring and performance management
* Reviewing clinical incidents, complaints and changes, providing solutions to reduce risks
* Leading compliance with AN-ACC assessments
* Managing person-centred care planning and evaluation for residents, including case conferencing with families and carers
* Fostering a strong team environment
* Preparing and conducting clinical governance audits
* Providing insights and innovative solutions for continuous clinical improvement
We're looking for a highly skilled and experienced Care Manager who can bring their expertise and passion for delivering quality clinical care to our team.
Key skills and qualifications include:
* Bachelor of Nursing degree with current APHRA registration
* Postgraduate qualification in aged care (highly regarded)
* Strong leadership, communication and interpersonal skills
* Person-centred care mindset
* Passion for nurturing a strong team environment
* Ingrained empathy for residents
* Proven track record in delivering quality clinical care in an aged care or acute setting
* Excellent clinical assessment and triaging skills, as well as strong care planning capabilities
