North and West Remote Health (NWRH) is currently seeking adaptable Journey Coordinators within the Stepped Care service system to join our team.
The Journey Coordinators are dedicated positions within the Stepped Care service system that operate as independent partners in a person's care journey. They may be standalone services, delivered within a Front Door service, or as an adjunct to other Stepped Care offerings. The role ensures individuals are at the centre of their own health and wellbeing, leading to improved care and mental health outcomes.
Your primary responsibility will be to deliver high-quality support and services by improving outcomes for Stepped Care participants. This includes building psycho-social education, supporting self-management approaches including digital mental health services and group programs, and facilitating the Stepped Care model to ensure the quality, continuity, and coordination of care between providers.
This is a full-time role based in Cairns and Townsville with outreach travel.

About you
We are looking for individuals with the following qualities and experience:
* Ability to establish, develop, and maintain productive relationships and networks
* Experience in developing partnerships and collaborations to enhance outcomes
* Understanding of mental health challenges and their impact
* Ability to develop genuine relationships with clients using a recovery-focused, person-centred, strengths-based, and trauma-informed approach
* Minimum Certificate IV in a mental health-related qualification (only those with suitable qualifications will be considered)
