This will be achieved working within the Prostate Cancer Foundation of Australia's framework and competency standards.On offer is a part-time opportunity working 4 days/week (Mon/Tues/Wed/Fri) on an initial 12-month maximum term contract.The role of the PCSN is to provide:Disease and treatment related care:Provide initial assessment of need using a systematic evidence-based approach.Maintain accurate records of nursing assessments and outcomes.Triage patients to determine areas of need and levels of assistance required.Maintain understanding of the rationale for the use of treatments and therapies used in the management of prostate cancer and their related clinical effects, both acute and delayed.Identify opportunities for patients to participate in clinical trials.Supportive care:Provide ongoing psychosocial support in accordance with individual needs of the patient.Act as a resource for contact, information, and support for those affected by cancer.Coordinated care:Coordinate a patient caseload and ensure the continuity of care.Ensure that key elements of care occur in an ordered and timely manner.Identify and make referrals to appropriate health and social agencies as per individual need.Ensure the transfer of patients between services within the area health service and external to the area health services when required.Information provision and education:Identify educational and supportive care needs of people affected by cancer and provide appropriate verbal and written information to meet those needs.Provide direct education to people affected by cancer within a range of settings.Support and empower patients to influence and participate in decisions concerning their care by providing information on a range of specialist care and services.Collaborative and therapeutic practice:Maintain communication and links with other members of the cancer care team.Support the delivery of multidisciplinary care and participate in multidisciplinary team (MDT) meetings or video conferencing as required.Advocate on patients' behalf including at multidisciplinary team meetings.Use effective communication skills to establish and maintain therapeutic relationships with people affected by cancer throughout the cancer journey.We’re looking for someone who has:Current nursing registration with AHPRA as a Registered Nurse division 1.Minimum of 5 years’ experience (preferred but all experience will be considered) and demonstrated knowledge and expertise in the care of people with prostate cancer.Demonstrated ability to coordinate and plan the care of people with prostate cancer across the continuum of care.Advanced interpersonal, communication and negotiation skills.Demonstrated commitment to quality of patient care.A proven ability to work effectively in a multi-disciplinary team.Word processing and computer skills.Desirable:Postgraduate Qualification in prostate cancer nursing, advanced nursing or a related field.An understanding of and experience with developing evidence-based practice and practice change experience.Demonstrated ability to develop patient/family education materials.Demonstrated involvement in professional activities and organisations including professional memberships.Demonstrate high level of skills in education and training programs for staff and actively participate in the ongoing learning and performance advancement of others by providing mentorship and clinical guidance.Demonstrate ability to work as a positive role model and mentor within the service.
