Proposed Roster - Fremantle Hub - 504296.pdf What you’ll do Support person-centred plans that identify a customer’s life goals and working to achieve them Join customers in the community and do recreational activities Support customers in daily living skills such as personal care, accompanying customers to medical appointments, attending activities of their choice such as shopping, lunching and movies. Ensure positive behaviour support plans are followed; and Administration duties and mandatory reporting What you’ll bring Well developed interpersonal skills with the ability to build effective working relationships; Willingness to assist with personal care and domestic duties experience Ability to follow Person Centred plans and assist people to achieve their goals and outcomes; An understanding and enthusiasm for working alongside people with disabilities; A valid drivers licence and willingness to use your private vehicle (aligned with genU motor vehicle allowance rebate).
