Key Responsibilities
* Provide person-centred support in line with individual support plans
* Assist with daily living activities, including personal care (where required)
* Support participants with community access and appointments
* Encourage independence, choice, and control
* Maintain clear shift notes and follow organisational policies
* Work collaboratively with the care team and management
Essential Requirements
* Female support worker (required due to participant needs)
* Certificate III or IV in Disability, Individual Support, or similar (or willing to obtain)
* NDIS Worker Screening Check (or willingness to apply)
* Working With Vulnerable People Check (WWVP)
* Current First Aid & CPR
* Valid Australian driver's licence and reliable vehicle
* Good communication skills and professional boundaries
Desirable
* Experience supporting participants with psychosocial or complex needs
* Experience in community access and supported accommodation
* Medication assistance experience (or willingness to train)
