About the Role:
We are currently seeking an experienced NDIS Service Coordinator to join our team on a temporary contract basis (38 hours per week) to provide leave coverage.
In this pivotal role, you will manage a caseload of up to four Supported Independent Living accommodations, supporting teams to deliver high-quality, person-centred supports. You will ensure participants are empowered to achieve their NDIS goals, exercise their human rights, and work towards personal aspirations.
Key Responsibilities:
* Lead and mentor teams to develop professional practice.
* Collaborate with other NDIS Service Coordinators to support participants in achieving their goals.
* Manage a complex caseload, ensuring support schedules align with the NDIS Price Guide, budget constraints, and NDIS Service Agreements.
* Prepare and submit NDIS Progress Reports in line with plan reviews and reporting requirements.
* Provide day-to-day leadership, supervision, and mentoring to staff to ensure high-quality service delivery.
* Maintain accurate client documentation, including risk assessments and support plans.
* Build and maintain strong relationships with key stakeholders including families, allied health professionals, and support coordinators.
About You:
You are a dynamic professional with a passion for empowering individuals and demonstrated experience in NDIS case management and/or support coordination. You are confident navigating the complexities of the NDIS framework and bring strong leadership and organisational skills to the role.
Key Selection Criteria:
* Demonstrated experience in NDIS case management and/or support coordination.
* Strong leadership skills with experience supervising staff in disability or community services.
* In-depth knowledge of NDIS standards, Human Rights, Person-Centred Active Support, and restrictive practices.
* Proven ability to build and maintain strong stakeholder relationships.
* Excellent organisational and time management skills, with the ability to manage competing priorities.
* Strong administrative and technology skills (client management, rostering, incident reporting).
* Relevant qualifications in Community Services, Disability, or a related field.
* Current Working with Children Check, NDIS Worker Screening, and Queensland Driver's Licence.
