We provide Supported Independent Living, In-home care, Community Participation, Short-Term Accommodation, Community Nursing, and Allied Health Services.
About the Role
We are looking for a female team member in our supported independent living arrangements in the suburbs of Redbank Plains and Deebing Heights to provide Supported Independent Living and Community Access.
Responsibilities
Supporting people with personal care and day-to-day activities in the home and out in the community
Providing positive behaviour support to our clients
Completing appropriate documentation through shift notes, incident reporting and data collection
Contribute to the team's overall objective of providing the best outcomes for our clients
Work with your team leader to implement and review plans around daily service provision
Maintain a professional relationship with clients and adhere to the NDIS and company code of conduct
Be responsive, reliable and accountable to our clients and your team

Qualification / Experience
Minimum Certificate III in Individual Support / Aged Care or Higher (or the ability to acquire an appropriate qualification)
Mental Health Experience

Verbal / Written Communication and Relationship Building Skills
