Senior Healthcare Leader
Aged Care Assessment is a multidisciplinary team that conducts comprehensive evaluations for frail clients to determine their care needs.
The team provides leadership, training, and support to clinicians while conducting assessments in hospital or community settings according to Commonwealth guidelines and the Aged Care Act 1997.
They focus on social work principles, comprehensive care planning, and ensuring quality, evidence-based practice.
Key Responsibilities:
* Provide leadership and training to clinicians
* Conduct assessments in various settings
* Develop comprehensive care plans based on social work principles
Requirements:
* Hold a Nursing qualification and maintain registration requirements with AHPRA
* Hold a tertiary qualification and maintain registration requirements with AHPRA
Previous experience in Aged Care Assessment would be an advantage.
