We are seeking an experienced Primary Health Nurse Practitioner to deliver advanced primary healthcare services within a correctional health setting across Victorian men's public prisons. This role provides high-level clinical care, leadership, and specialist advice, working collaboratively within a multidisciplinary team to ensure equitable healthcare outcomes comparable to community standards. This position may suit an endorsed Nurse Practitioner or a Transitional Nurse Practitioner progressing toward AHPRA endorsement. Key Responsibilities Deliver nurse practitioner-led primary health clinics (face-to-face and via telehealth) Triage referrals and coordinate care with medical officers and multidisciplinary teams Order, review and interpret pathology and diagnostic investigations Prescribe and adjust medications within scope and legislative requirements Provide expert clinical advice and leadership to nursing staff Manage complex and chronic health conditions and medical emergencies Coordinate discharge planning and post-release continuity of care with community providers Maintain accurate clinical documentation within electronic medical records Participate in on-call rosters, clinical governance, quality improvement and accreditation activities Contribute to policy development, education, mentoring and supervision About You Current AHPRA registration as a Registered Nurse and Nurse Practitioner (or evidence of progress toward NP endorsement for Transitional applicants) Minimum 5 years' post-registration nursing experience with advanced clinical practice Strong background in primary health care, chronic disease management and emergency care Excellent communication, leadership and clinical decision-making skills Commitment to ongoing professional development and clinical governance Postgraduate qualifications in Nurse Practitioner studies (completed or in progress) Additional Requirements Ability to work in a correctional health environment. Current Australian driver's licence. National Criminal History Check and security clearance eligibility. Availability for after-hours and on-call work as required.
