Key Responsibilities:
* Develop and oversee re-enablement programs to empower clients
* Collaborate with multidisciplinary teams, including occupational therapists, physiotherapists, nursing staff, and therapy assistants
* Identify and implement tailored solutions to help clients achieve their goals
This community-based role covers suburbs from Peel to Secret Harbour and Lake Clifton, with some coverage in the Rockingham region. You will have the opportunity to work closely with clients and make a meaningful difference in their lives.
