About the Role:The Placement Coordinator is responsible for actively supporting, initiating, and coordinating appropriate permanent placement options for older adults participating in the Time to Think (T2T) program following their discharge from hospital.
The role ensures that each participant's transition from hospital into longer-term care—whether residential aged care, home-based supports, or alternative living arrangements—is safe, timely, person-centred, and informed by clinical, functional, and social assessments.
The Placement Coordinator works collaboratively with hospital teams, T2T nursing staff, allied health professionals, medical practitioners, My Aged Care assessors, aged care providers, participants, and their families to determine the most suitable long-term placement that meets the participant's needs, preferences, and goals.
Key Responsibilities:
Placement Coordination & Planning
* Coordinate all aspects of the participant's permanent placement pathway from T2T admission through to discharge, in consultation with the Residential Care Manager and multidisciplinary team.
* Engage participants and families in discussions regarding care options, preferences, timelines, decision-making processes, and placement recommendations.
* Liaise with aged care providers to identify suitable vacancies aligned with participant needs, clinical assessments, and care plans.
* Facilitate referrals to appropriate services, including Residential Aged Care, Support at Home & Restorative Care, Community Support, and Retirement Living.
* Coordinate placement into the agreed care environment once final recommendations are confirmed.
Stakeholder Engagement & Collaboration
* Act as a key liaison between the Aged Care Hub, Capecare T2T staff and management, My Aged Care, ACAT/RAS assessors, community services, and aged care providers.
* Build and maintain strong relationships with residential aged care facilities to remain informed on availability, service models, and admissions processes.
* Collaborate with geriatricians, social workers, allied health, and clinical teams to ensure effective communication and handover documentation.
* Provide regular progress and placement updates to participants, families, and relevant stakeholders.
Participant & Family Support
* Support participants and families to navigate financial and administrative processes, including Centrelink income and assets assessments, means testing, and RAD/DAP expectations (information only).
* Coordinate facility tours and provide guidance to support informed decision-making.
Administration, Reporting & Continuous Improvement
* Maintain accurate, timely documentation within organisational systems and T2T client records.
* Prepare reports, summaries, and progress updates for internal and external stakeholders.
* Monitor and report on outcomes such as placement acceptance rates, discharge destinations, wait times, and participant satisfaction.
* Contribute to continuous improvement initiatives to enhance T2T discharge and placement pathways.
To be successful in this role, you'll need:
* Qualifications in Business Administration or similar would be beneficial
* Demonstrated experience in aged care assessment, hospital discharge planning, or care coordination would be seen as beneficial.
* Demonstrated experience in coordinating high volumes of referrals or case loads.
* Knowledge of the Australian aged care system including Residential Aged Care, Support at Home Packages, My Aged Care referral pathways, ACAT assessments, and Transition Care Program or similar frameworks would be highly beneficial.
* Excellent verbal and written communication skills.
* Well-developed documentation and reporting skills.
* Commitment to continuous improvement and high-quality care.
* Australian work authorisation is required at the time of application.
* Police check clearance is required for this position.
