You will provide personal care, health monitoring, domestic assistance, transport and reablement care to community rehabilitation clients. Work as part of a dedicated, caring and positive team that offers training and professional support.* Provide care and supports to Transitional Aged Care Program clients in their own home and in the community.* Assist clients with personal care, health monitoring, socialisation, transport, shopping and domestic duties.
