You will provide personal care, health monitoring, domestic assistance, transport and reablement care to community rehabilitation clients. Work as part of a dedicated, caring and positive team that offers training and professional support.* Provide care and supports to Transitional Aged Care Program clients in their own home and in the community.* Assist clients with personal care, health monitoring, socialisation, transport, shopping and domestic duties. Enjoy a role that provides variety, challenge and is rewarding.* Work as part of multi-disciplinary team striving to provide excellent patient centred care and rehabilitation services.* Set shifts that offer predictability and stability; accrue sick leave and annual leave.* Professional development supported and supervision provided.**What you'll bring**You're resilient, compassionate and have a whole lot of heart.
