As a Supportive Care Case Manager at the Leukaemia Foundation, you'll support people and families impacted by blood cancer through compassionate, personalised care. You'll provide emotional, practical and information support via phone, video, face-to-face and digital channels, helping improve quality of life and wellbeing.
This is a Monday to Friday role with flexible hours, no nights, weekends or on-call, and the freedom to plan your week across office-based work and hospital visits. You'll also be supported with onsite training and ongoing CPD to maintain and grow your professional practice.
About the role
* Permanent part-time (30 hours per week) with flexible days and hours
* Based at our Adelaide (Lightsview) office with free onsite parking
* Option to work from home one day per week
* Part of a national Clinical Services team
What you'll do
* Deliver high-quality supportive care and coordinate services for people impacted by blood cancer
* Empower clients and families to manage their physical, emotional, social and financial wellbeing
* Promote understanding of blood cancer and available support options
* Maintain professional, regulatory and documentation standards
* Contribute to continuous service improvement
About you (essential)
* Tertiary qualification in nursing, social work or allied health
* AHPRA registration or eligibility for relevant professional membership
* Strong communication skills and a compassionate, client-centred approach
* Excellent organisation, time management and IT skills
* Ability to work independently and within a multidisciplinary team
* Emotional resilience and commitment to continuous professional development
* Current driver's licence and access to a vehicle
* Working with children/vulnerable people clearance (or willingness to obtain)
* Documented evidence of immunisation against Measles, Mumps, and Rubella (MMR), Pertussis (Whooping Cough) & Varicella (Chickenpox).
* Experience in haematology, oncology or cancer supportive care
* Knowledge of community cancer support services
* Counselling or psychosocial support training
* Eligibility for a Medicare provider number
