The Royal Melbourne Hospital Transition Care Program (Home based) is a jointly funded program (State & Commonwealth) that aims to assist frail aged patients to complete their restorative processes at home, optimise their functional capacity and finalise their long-term care arrangements. About The Role The Transition Care Program Home Based Team leader is a key leadership position that supports quality planning, policy review, staff supervision and ensures services are aligned with legislation, organizational objectives and professional standards. The role works closely with the clinical coordinator for the program to ensure appropriate client service provision and flow. About You You are an Allied Health Professional or Registered Nurse under the Australian Health Practitioner Regulation National Law Act (2009) You have significant experience working in multi-disciplinary teams in hospital and/or community settings including working as an Aged Care Assessor, Case Manager / Care Coordinator and/or in leadership or management roles You are a flexible, energetic, and innovative leader with comprehensive understanding of the Aged Care system and the Commonwealth Aged Care Reforms. You have highly developed knowledge of Aged Care and Community Services including the Transition Care Program, Aged Care Assessment, Commonwealth Home Support Program (CHSP), Support at Home, Residential Care and Respite. Demonstrated organisational skills and ability to prioritise work whilst maintaining a commitment to quality of care and best practice.
