As part of the clinical team, you will collaborate closely with the Home Care Team to manage aspects of clinical care.You will oversee a team of nurses and support workers, ensuring high standards of patient care.
Your responsibilities include overseeing daily operations, developing and implementing policies, and coordinating patient care with healthcare professionals.This role involves clinical assessments, providing ongoing clinical support and advice, and direct clinical care when necessary.
You will monitor health declines and develop, review, and maintain individualized support care plans for Home Care Clients, respite, and short-term accommodations.You must ensure the efficient delivery of professional care, respecting clients' rights, in consultation with clients, families, guardians, and key health professionals.The role also involves training, leadership, supervision, and clinical support to the Home Care Team, ensuring compliance with professional standards and legislative requirements.Main Tasks of the Complex Nurse Coordinator...Client Clinical CareConduct assessments, manage care, coordinate with external providers and agencies.Create, monitor, and review care plans.Meet high-intensity care requirements (wound, respiratory, medication management, dysphagia, etc.
).Liaise with and refer to external service providers and medical practitioners as needed.Administrative ResponsibilitiesHandle case notes, review client records, monitor concerns or decline, and provide reports.Manage incidents and mandatory reporting.Create, review, and update policies and procedures.Develop promotional plans for the Home Care Program and other services.Interpret legislation and stay updated on industry and government changes.Staff Supervision, Education, and TrainingSupport staff regarding clients' care plans.Provide training and conduct competency assessments for staff and external providers.Service Planning & DevelopmentContribute to service planning and improvements.Maintain contact with clients' primary health teams and organizations.Stay informed about policies, government schemes, and best practices; recommend actions.Ensure regulatory compliance.Qualifications & Experience...Current Nursing Registration with AHPRA.Experience in acute health, community health, complex case management, or palliative care.Certificate IV in Training and Assessment (highly desirable).Ability to meet deadlines, manage priorities, delegate effectively.Flexible working hours to cover shifts and events.Experience mentoring and supervising staff.Experience working with families and commitment to family-centered care (preferred).Skills & Attributes...Commitment to child-safe practices.Reliable, hardworking, self-motivated.Strong interpersonal, communication, and computer skills (Microsoft 365, Teams).Ability to work independently and in teams.Ability to build relationships with stakeholders such as hospitals, specialists, GPs, allied health, families, and volunteers.Salary packaging up to $15,900 tax-free, Meal Entertainment up to $2,650 tax-free, Novated Leasing or Equilease, 12.5% superannuation, Paid Bonus Schemes.This description illustrates the role's scope and responsibilities but is not exhaustive.
