Enterprise Agreement: The Named NSW (Non-Declared) Affiliated Health Organisations Nurses Agreement 2017.Role descriptionTresillian2U Early Years Parenting Service (T2U) is provided from a fit-for-purpose van.
The T2U service will visit towns of lower population density within a designated area on a rotational basis providing individual consultations, group programs and community engagement activities.
This unique service model provides greater access to care and support for families experiencing complex early parenting difficulties, while being connected for clinical support through the Tresillian network of services.
The successful applicant will work as part of a team with an Aboriginal Health Worker, establishing key connections and opportunities for collaboration with local service providers to improve the pathways to care and support for families.This position will be involved in the provision of quality child and family health services to families caring for children 0-3 years of age within a Tresillian service.
This will be achieved by working within the Tresillian Model of Care, Practice Principles and the NSW Health Professional Practice Framework for Child and Family Health (CFH).
The CFH clinician will use a strength-based and wellness focus, building on parental confidence and skills through the provision of information, education, resources, guidance, and role modelling safe responsive care.
The child and family health nurse, utilising evidenced based screening and assessment tools, uses their family partnership skills to identify health variations or family vulnerabilities to negotiate individual care plans and discharge pathways to meet the goals of the family.ResponsibilitiesProvide quality child and family health services to families with children aged 0-3 years within a Tresillian service.Work within the Tresillian Model of Care, Practice Principles and the NSW Health Professional Practice Framework for Child and Family Health (CFH).
