Overview

YAMHS Transitioning of Care Coordinator - Identified position at Bendigo Advertiser. This is a temporary full-time role until June 2026, based at the Mater Mental Health Centre, Waratah (Awabakal Country). Salary: $77,358 - $133,156 per annum + super. Hours per week: 38.

Note: This is an identified position for Aboriginal or Torres Strait Islander people under the Anti-Discrimination Act 1977. Aboriginal or Torres Strait Islander applicants must demonstrate status in addition to addressing the selection criteria. YAMHS is a stand-alone Aboriginal mental health service that provides transitioning of care coordination, culturally grounded care, and holistic mental health support.


About Us

YAMHS is a standalone Aboriginal mental health specialty service that supports Aboriginal and Torres Strait Islander people across the lifespan. Delivered by Mob, for Mob, the service partners with Aboriginal communities to ensure care is culturally grounded and community-informed. The team focuses on healing, wellbeing, cultural safety, and clinical excellence within HNEMHS district.


Responsibilities

* Provide Transitioning of Care Coordination (TOCC) wrap-around support.
* Perform culturally grounded mental health assessment & care planning.
* Coordinate care management, therapeutic interventions, and intensive follow-up interventions.
* Deliver healing supports, clinical yarning, and cultural consultation.
* Collaborate with patients, families, and communities; deliver care across the lifespan as specialist mental health clinicians.
* Support transitions to and from Calvary emergency department to mater PECC, MHICU, and mental health inpatient units / community settings for Aboriginal and Torres Strait Islander consumers.
* Provide cultural consults and initial MH assessment with YAMHS CNC/TOCCs; work in a cultural trauma-informed, recovery-focused framework.


About You / Requirements

* Completed degree/qualifications in Bachelor of Health Science Mental Health (Djirruwang program), Nursing, Social Work, Occupational Therapy, or Psychology with membership of (or eligibility for) the relevant professional association and/or current registration with AHPRA, or as an Aboriginal Health Worker with appropriate eligibility.
* Clinical Nurse Specialist Grade 2 requires relevant post-registration qualifications and at least 3 years' experience in the clinical area.
* Eligibility to drive in NSW and willingness to drive/travel as the role may involve long distances and overnight stays.
* Ability to work independently and as part of a multidisciplinary team; strong skills in mental health assessment, risk assessment, and treatment planning.
