Our origins are in Gippsland regional Victoria, but we have grown rapidly in recent years. Our services include general practice (GP); nursing; allied health; dental; counselling and psychology; gambling support; alcohol and drug treatment; migrant and refugee settlement services; social connection; carer support services and National Disability Insurance Scheme (NDIS) in partnership with the NDIA.

Total remuneration package of $99,965 - $108,781 per annum including superannuation and the full optional benefit of salary packaging.

What you’ll be doing in this role?

The responsibilities of a Care Partner are varied and each day brings new challenges. As a member of a larger team, you will oversee a portfolio of clients, ensuring that you provide the necessary support for them to safely stay at home while working towards their goals.

To be successful, you will require:

- Assessment skills: A care advisor is responsible for conducting thorough assessments of individuals' needs, including their physical, emotional, and social well-being. They gather information about the person's condition, medical history, support network, and any specific requirements to determine the appropriate level and type of care.
- Care Planning: Based on the assessment, a care advisor develops comprehensive care plans tailored to each individual's unique needs and preferences. They consider factors such as medical treatments, personal assistance, emotional support, and social activities to ensure holistic care.
- Consumer-Focused: A care advisor places the consumer at the center of their work, prioritising their choices, preferences, and goals. They actively involve the individual in the decision-making process and empower them to make informed choices about their care.
- Liaison with Health Professionals: As a liaison between the consumer and the healthcare system, a care advisor communicates and collaborates with various health professionals such as doctors, nurses, therapists, and social workers. They ensure that the consumer's care plan is effectively implemented, and any necessary adjustments or referrals are made.
- Visiting Consumer: Care advisors often visit consumers in their homes to assess their well-being, monitor the progress of their care plan, and provide ongoing support. These visits allow them to build rapport, address concerns, and ensure the quality of care being delivered.
- Empathy and Communication: A care advisor needs to possess robust empathy and communication skills. They should be able to listen actively, understand the needs and emotions of the consumer, and communicate effectively to build trust and establish a positive relationship.
- Problem-Solving and Advocacy: Care advisors need to be skilled at problem-solving, identifying challenges, and finding appropriate solutions. They advocate for the consumer's rights, ensuring they receive the best possible care, and navigate any obstacles or barriers that may arise.
- Organisation and Time Management: Care advisors often handle multiple cases simultaneously. Therefore, they need excellent organisational skills to manage their workload, prioritise tasks, and meet deadlines effectively.
- Compassion and Patience: Above all, a care advisor should have a compassionate and patient approach when working with individuals who may be experiencing physical or emotional challenges. They should provide reassurance, encouragement, and emotional support to help individuals and their families navigate difficult situations.

What we’re looking for :

- Appropriate qualifications compliant with Australian Qualifications Framework (e.g., Diploma of Case Management and/or Community Services/Nursing or related field)
- A current Victorian Drivers Licence
- A current Working with Children Check (Employee) or VIT Registration
