Job Overview

Key Responsibilities
* Collaborate with clients and their families to provide high-quality clinical care.
* Support admissions and transitions of care for clients requiring home-based services.
* Undertake comprehensive assessments of clients' needs and develop personalized care plans.
* Provide wound care, continence management, palliative care, medication management, and restorative care to meet clients' physical and emotional needs.
* Liaise with multidisciplinary teams to ensure seamless service delivery and achieve optimal outcomes for clients.
* Assist with education and training for colleagues as required.
Requirements and Qualifications
* AHPRA registration and current practicing certificate.
* Availability to work Monday to Friday with occasional weekend shifts.
* Aged care experience is an asset.
* Excellent verbal and written communication skills.
* Genuine enthusiasm and compassion for working with older adults.
* Certification in CPR and First Aid is highly desirable.
