Your focus will be on understanding each individual's unique needs and circumstances, particularly those from diverse and vulnerable backgrounds, including LGBTQI individuals, culturally and linguistically diverse (CALD) communities, and Indigenous populations.You'll ensure clients' voices are heard and respected in the care planning process, fostering an inclusive and supportive environment.Your work will include:Conducting person-centred, non-clinical assessments to identify aged care support needs.Implementing Reablement strategies and Linking Service Support to promote independence.Collaborating with clients, families, and service providers to ensure seamless access to appropriate services.Engaging with the community and contributing to quality improvement initiatives.Why join our Assessment Services team?We are developing a flexible workforce to meet the growing community need for aged care assessments.
