As a Connections Support Officer, you'll help ensure our teams and local clinicians have the information they need to deliver coordinated, high-quality care. Every day, you'll contribute by:
Coordinating the ongoing development and review of clinical pathways to keep information accurate and useful.
Working closely with clinicians, partners and service providers to gather insights that strengthen shared decision-making.
Supporting collaborative groups and review processes that drive improvements in how care is delivered.
Enhancing secure communication channels with general practice to improve connection and service visibility.
