The Community Dementia Clinical Nurse Consultant (CNC) is a Registered Nurse who has responsibility for providing expert clinical advice, coordination, and consultancy services, to patients with cognitive impairment, carers and other health professionals relating to complex health care needs. The CNC’s primary purpose is to improve outcomes for people with cognitive impairment, their families, and carers in the community.
The Clinical Nurse Consultant is a clinical expert in Dementia and cognitive impairment who, through leadership, facilitates excellence in evidence based clinical practice, education, research, and professional leadership.
The Clinical Nurse Consultant is part of the multidisciplinary Community Assessment and Therapy Team in Prince of Wales Community health. The CNC works with clinical teams to develop, facilitate the implementation of, and evaluation of care management plans for patients diagnosed with or experiencing cognitive impairment related conditions. The role is focused on integrating care and acting as a resource across Community Health, Prince of Wales Hospital and SESLHD.
Clinical Nurse Consultant is employed as grade 2 according to the criteria outlined below, and in line with the NSW Public Health System Nurses’ and Midwives’ (State) Award.
Clinical Nurse Consultant Grade 2 means: a registered nurse appointed as such to a position approved by the Health Service, who has at least 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field. In addition the employee must have approved post graduate nursing qualifications relevant to the field in which he/she is appointed to or such other qualifications or experience deemed appropriate by the Health Service. An employer may also require a higher qualification in the specialist nursing field where such qualification is considered essential for the performance of the individual position.
Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified for this position following suitable labour market testing.
All workers and new recruits are required to receive 2 doses of a Therapeutic Goods Administration approved or recognised COVID-19 vaccine to commence employment/ engagement or continue to work within a NSW Health service.
A worker and new recruit will be considered compliant if they have a medical contraindication to all available Therapeutic Goods Administration approved or recognised COVID-19 vaccines and provide medical contraindication evidence in line with the policy requirements.
In addition, all Category A workers and new recruits are required to receive one dose of the seasonal influenza vaccine annually to be considered compliant.
Category A workers and new recruits who are non-compliant with seasonal influenza vaccination or have a medical contraindication to influenza or COVID-19 vaccinations must comply with all other infection control risk reduction strategies as directed while working in a Category A position.
