Key Responsibilities:
* Provide comprehensive support to individuals with dementia
* Organise a Clinic for Dementia and Aged Care alongside a Rehabilitation Specialist
* Provide vital support through home visits
You'll work independently and within a collaborative, multidisciplinary team to deliver person- and family-centred care. We're looking for someone with excellent communication, assessment, and time management skills who can connect with people from diverse cultural backgrounds.
The ideal candidate will possess current registration with AHPRA as a Registered Nurse and proven clinical experience in dementia, delirium, or aged care case management.
