The Salvation Army (Salvos) Youth Services encompasses a range of programs for vulnerable young people, their families, and the community. Our services have a strong focus on prevention and early intervention and enable access to safe accommodation options, Holistic case management, community outreach programs and programs that teach vital independent living skills. About the role
As a Case Manager, this is your opportunity to provide Intensive case management to young people between the ages of 16 and 18 years of age, under a current Guardianship of the Chief Executive orders through the Department for Child Protection
Reporting to the Team Leader, this is a permanent, full-time position, based in Whyalla, SA.
How you will make an impact
- Support is provided to young people in line with Youth Services Model of care and associated Frameworks
- Effective relationships are established with clients that encourage self-determination and participation
- Case management support is provided to clients in accordance with TSA case management models and standards of practice
- Ongoing assessment is made of the client's issues, needs and circumstances from initial intake to exit
- Individual case plans are developed in collaboration with clients and include a range of interventions that address the needs or goals of clients
- Case plans are implemented with active client participation
What you will bring
- Degree qualification in social work, welfare, community development or related field
- Relevant experience in a social service environment
- Working with Children Check
- Psychological Assessment (in line with Children and Young People Safety Act 2017)
- A national police record check is required
- Current Drivers License
- First Aid Certificate
- Through their Eyes (Reporting Abuse and Neglect) Training
