The social worker liaises with the multidisciplinary team and external stakeholders to facilitate safe and timely discharge from hospital. The social worker is responsible for overseeing and facilitating applications to the NDIA and other community based services. The social worker provides assessment and intervention in relation to risk where elder abuse, child protection, domestic violence and trauma may be present and provides applications to NCAT on behalf of the hospital when a patient is deemed to lack capacity and decisions regarding their care. What you'll be doing To provide Social Work services to consumers and their carers to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct. The position will also participate in professional development, clinical supervision, teaching, quality activities and other relevant departmental and administrative duties. Where You'll Be Working Liverpool Hospital, founded in 1813, is the major health service for south-western Sydney, providing services to the local government area of Liverpool City Council as well as district services to residents and visitors in the area. It also provides a range of state-wide services in areas such as critical care and trauma, neonatal intensive care and brain injury rehabilitation. The Hospital provides a health service of international standing, with 23 operating theatres, capacity for 877 beds, diagnostic and imaging services, emergency and trauma care, maternity, paediatric, cancer care, mental health, ambulatory care, allied health and medical and surgical services from birth to aged care. Liverpool Hospital is currently undergoing a major redevelopment. The $790 million Liverpool Health and Academic Precinct (LHAP) project will provide enhanced facilities and an increased capacity to meet future significant population growth for South Western Sydney. A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW). Demonstrated experience in a health related field and ability to provide a range of Social Work interventions. Demonstrated experience prioritising work and managing stressful situations. Proven ability to work independently and collaboratively within a multi-disciplinary team. Demonstrated high level communication skills (written and verbal) and computer skills. Evidence of commitment to clinical supervision and professional learning.
