**Job Title:** Physiotherapist / Occupational Therapist (Transition Care)
This role involves providing clinical rehabilitation therapy to clients on the community rehabilitation program.
* Manage cases for clients in the community rehabilitation program.
* Provide physiotherapy or occupational therapy activities within the multidisciplinary team.
* Supervise and support therapy programs with the allied health assistant.
* Participate in quality activities within the team.
**Key Responsibilities:
1. Clinical assessment, diagnosis, and management of client conditions.
2. Development and implementation of individualized therapy plans.
3. Maintenance of accurate records and reporting.
4. Collaboration with other healthcare professionals to ensure comprehensive care.
**About You:**
To be successful in this role, you must be a registered physiotherapist or occupational therapist with AHPRA and any relevant endorsements.
