About The Role:
Port Lincoln Aboriginal Health Service is seeking a dedicated and proactive Cancer Care Support Worker to join our team on a fixed-term basis until June 2027.

The Role
As part of the Cancer Care team, the key responsibilities of the Cancer Care Support Worker will include but is not limited to:
* Effective communication and reporting with all key working relationships
* Work in collaboration with the other Cancer Care Support Worker and Registered Nurse – Cancer Care to provide education and support with preventative cancer screening and completion of screening
* Create awareness and deliver education to community members to encourage positive behaviour change by planning, developing and delivering health promotion activities using a variety of resources and strategies. These programs and resources will be co-designed, locally relevant and culturally safe and responsive with the intention of increasing the adoption of health promoting behaviours and screening participation in Aboriginal and Torres Strait Islander communities.
* Assist with follow up / referrals as appropriate
* Support diagnosed patients navigate the cancer care pathway and assist with developing a comprehensive care plan which includes assessment, planning, treatment, supportive care, survivorship/palliation in collaboration with team members and other service providers ie Palliative Care
* Collaborate with other teams at PLAHS and act as liaison between patients, family, general practitioners, specialists, hospital and community providers
* Assist Aboriginal and Torres Strait Islander peoples navigate and liaise with cancer services across all aspects of cancer care
* Attend case conferences and clinical meetings, ensuring timely communication of client needs
* Accompany community members to cancer screening, diagnostic and treatment appointments and liaise with medical staff as required
* Provide ongoing supportive care between appointments, including facilitating access to medicines, food and other necessities, as well as arrange financial and psychological support
* attending and contributing to family meetings and case conferences and facilitate communications between the Community member, the community-controlled organisation, mainstream treating teams and families
* Support patients with developing Advance Care Directives or Advanced Care Plans as appropriate and coordinate end of life care in coordination with other services
* Contribute to quality improvement activities, data collection and program reporting
* Meet key outcomes and KPI requirements of the NACCHO Cancer Support Program
* Coordinate cancer services and supports accessible to clients with cancer
* Improve access to cancer care services and supports
* Ensure cancer patients are provided with holistic care, including ensuring access to internal/external psychological services/counselling as appropriate
* Ability to work autonomously and within the multidisciplinary team and demonstrated commitment to ongoing professional development
* Work collaboratively with Aboriginal Health Practitioners, Aboriginal Health Workers and all other health care providers and support workers
* Contribute to the planning, development, implementation, monitoring and evaluation of program strategies and resources to meet the health and wellbeing needs of the Aboriginal community.
Essential Qualifications, Skills and Experience
* Demonstrated ability to work effectively in a multi-disciplinary team approach
* Ability to communicate effectively with both Aboriginal and non-Aboriginal people in a culturally sensitive manner via an appropriate variety of methods
* Ability to develop, keep and maintain accurate case files and client records
* Demonstrated ability in using initiative, working independently with minimal supervision, determining priorities, meeting deadlines, planning and organising work.
* Ability to effectively utilise information technology, data entry and Patient Information Recall Systems, in particular Communicare, and Microsoft Office products.
* Willingness to undertake training/staff development as required
* Experience in working effectively with Aboriginal and non-Aboriginal people.
* Experience working in a multidisciplinary team to enhance service delivery through supporting staff expertise and morale
* Experience in providing person-centred support to enhance individual, family and community strengths, resilience, health and quality of life
* Experience in developing and successfully delivering a planned schedule of health promotional activities
* Knowledge, understanding and awareness of the diversity of Aboriginal cultures and societies
* Knowledge and understanding of Cultural Safety principles and their workplace implementation
* Knowledge of Social Justice principles and practice
* Knowledge of Equal Opportunity and Work Health & Safety (WHS) Legislation
* Knowledge of relevant local resources and organisations
* Knowledge of client-centred, consultative and community development approaches
Requirements:
* Must hold a valid, unrestricted driver's licence
* A current DCSI Working with Children Check (WWCC) issued within the last 5 years
* A National Police Certificate issued within the last three years.
* Evidence of compliance with PLAHS staff immunisation requirements including an annual influenza vaccination.
