Your role at St John of God Bendigo Hospital
You will be responsible for the provision of high-quality diabetes education clinical services to clients, education to team members relating to the delivery of services, and co-ordination of the service.
The Position
Provide an efficient service for inpatients and outpatients on both a routine and urgent basis providing high quality clinical outcomes.Educate, assist and support people with diabetes and their families on promoting euglycaemia.Notify Medical Practitioners of patients' condition and their skills in the management of their condition.Liaise closely with the Dietitian in the care of people with diabetes.Review all hospital newly diagnosed diabetics after discharge by telephone.Ensure all inpatients' needs are assessed and they are equipped with the appropriate equipment for discharge to minimise the need for continued support post-discharge.Ensure appropriate records are kept on all patients attended.
Minimum Requirements
You will hold a registration with the Australian Health Practitioner Regulatory Agency (AHPRA) and be a Credentialed A.D.E.A. member (or working towards).
To succeed you will demonstrate continuing education and commitment to the provision of quality health care and demonstrate leadership skills and ability and willingness to mentor and coach caregivers.
