What makes this role special?
Community aged care reforms are opening new doors for Nurses who want to make a real difference. · Proactively track client health and well-being, identifying changes and facilitating timely interventions and referrals.
· Empower clients to make informed decisions about their care, respecting their preferences and promoting independence.
· Provide clear guidance to clients and families on health management, medication, and lifestyle choices.
· Maintain accurate, compliant records of assessments, care plans, and interventions.
What We're Looking For

· Values the right mindset, adaptability, and a genuine passion for positive ageing.
