Apply on Kit Job: kitjobau.com/job/38d3rw About the Team The School Based Youth Health Nurse Service aims to promote positive health outcomes for young people and their families through the provision of a primary health care service centred around the principles of early intervention, assessment and harm minimisation and prevention. The service provides, the prospect for young people, their parents/carers, and members of the school community to access a health professional in the school setting for matters relating to the health and well-being of young people. Nurses provide individual health consultations to support the health literacy and help seeking behaviours of young people. The service is delivered in partnership with the Department of Education to state secondary school students. Nurses play a key role in supporting school communities to adopt a whole- school approach to wellbeing, utilising the health promoting school's framework to address emerging health and social concerns. This approach supports young people in making a safe and healthy transition into adulthood. About the Role The Clinical Nurse (CN) role provides advanced clinical and problem-solving skills, expert planning and coordination skills in the clinical support of young people with complex care needs, ensuring quality standards are met. This role works autonomously within the Healthcare Team structure and is pivotal in the planning, managing and delivering of specialist youth health nursing care. It integrates the social model of health and applies primary health care principles with the secondary school environment. The CN provides clinical leadership for professional and clinical practice, education and research. The role supports in management activities including portfolio responsibilities. Autonomy Practices autonomously. Provides leadership in clinical decision making to give and/or coordinate care to young people. Assumes responsibility for professional leadership for a ward, service or unit in the absence of the Nurse Grade 7 (NG7). Applies critical thinking across all the domains.
