The Pathways to Recovery program supports individuals with severe and complex mental illness through collaborative care coordination. Working alongside Peer Support Workers will be critical in the client recovery journey.
The Role
The Clinician plays a key role in delivering focused psychological interventions, coordinating case management services for clients with complex mental health concerns in primary care by:
* Collaboratively planning and implementing assessment and treatment services for individual clients.
* Providing goal-oriented therapeutic interventions and strategies for eligible clients.
* Working effectively as part of a multidisciplinary team.
About You
* A genuine caring nature with a passion to help others is essential.
* Excellent communication skills are necessary to build meaningful relationships.
* Qualifications suitable for appropriate registration include Social Worker, Psychologist, Provisional Psychologist, Occupational Therapist, or Registered Nurse (RN).
* Minimum two years full-time post-graduate experience working within primary/community mental health settings or equivalent is highly desirable.
* Some out-of-hours work and intra-state travel may be required.
