Fixed Term Part Time – 68 hours per fortnight (negotiable)

Available from 9th February 2026 to 29th May 2026

An exciting and rewarding opportunity is available for a suitably qualified Social Worker to join the Allied Health team at Portland District Health.

In the role of community Social worker you are responsible for delivering safe and effective clinical care that requires a higher level of clinical decision making that is relevant to social work practice. You practice and deliver social work services across acute, sub-acute, outpatient and community-based settings, and by negotiation for the public funded residential aged care facility, within Portland District Health.

As PDH Welfare worker you will provide practical assistance with housing referrals, Centrelink application/support, NDIS application support, VPTAS claims. As part of your role, you may undertake carer assessments and referrals to community providers / services. You may also assist consumers and or their family / caregivers to apply to My Aged Care for services / supports.

Role may also include some hours in intake role.

About you:

* Possession of a tertiary qualification in social work with membership or eligibility of membership with Australian Association of Social Workers
* Possession of a Qualification in Welfare Studies with membership or eligibility of membership with Australian Institute of Welfare and Community Workers (AIWCW).
* Demonstrated and significant level of clinical knowledge, skills, and leadership experience in social work
* Demonstrated clinical assessment and analysis skills and knowledge, and ability to implement indicated treatments for individuals and groups.
* Knowledge and understanding of the role of Welfare Officer in an acute / sub-acute rehab setting an or community setting.
* Demonstrated ability to provide a Welfare service, including knowledge and experience in assessment for services.
