Commonwealth Home Support Program (CHSP) Coordinator
About the Role
As the CHSP Coordinator, you’ll play a pivotal role in designing and delivering service packages that are commercially minded, creative, and client focused. You’ll think outside the box to tailor solutions that are affordable, effective, and high-quality, ensuring our clients receive the best possible support.
You’ll also bring strong administrative capabilities to the role, confidently managing billing, co-contributions, and reporting in line with government guidelines. Your key responsibilities will include:
- Building and maintaining trusted relationships with clients, RAS assessors, referral agencies, and allied health professionals.
- Creating and coordinating service packages that meet client needs while aligning with CHSP targets and funding requirements.
- Monitoring service outputs and ensuring compliance with CHSP guidelines.
- Managing bookings, billing, and reporting with accuracy and efficiency.
- Supporting clients with complex needs and escalating feedback appropriately.
- Promoting Lutheran Services as a provider of choice within the local community.

About You

- Experience in aged care, home care, or case management.
- Strong administrative and customer service skills.
- Excellent communication and stakeholder engagement abilities.
- Knowledge of CHSP guidelines and aged care legislation, or a willingness to learn.
- The ability to work autonomously and manage competing priorities.
- A current driver’s licence and a valid police check.
