Customer-Centric Respite Care Coordinator
We are seeking an experienced and compassionate individual to join our team as a Customer-Centric Respite Care Coordinator.
About the Role:
* This is a part-time position, servicing Geraldton.
* The successful candidate will work in our Home and Community Care team, providing services under various funding programs.
Responsibilities:
* Develop and implement a Model of Care that prioritizes customer-centric quality care services.
* Manage a prescribed caseload, including assessment and care planning.
* Ensure all reporting and documentation requirements are met.
* Provide coordination, assessment, care planning and review for non-clinical components of support programs.
Requirements:
* Certificate IV in Leisure and Lifestyle, Diversional Therapy or similar qualification deemed equivalent.
* Certificate III in Aged Care or equivalent.
* Current First Aid and CPR Certificate.
* Excellent computer skills with demonstrated experience in MS Office.
* Strong understanding and knowledge of client needs from various funding programs.
