Make a real impact on the health of children in Out of Home Care. Work closely with diverse teams across health and community sectors. Lead care planning in a vital child wellbeing initiative in WSLHD. Support the health of kids in care! Coordinate assessments & plans in a vital liaison role across agencies in Western Sydney. Employment Type: Temporary Part Time (Up to 30/06/2028) Position Classification: Clinical Nurse Specialist Grade 2 Remuneration: $57.11 - $58.98 per hour Hours Per Week: 24 Location: Mount Druitt Hospital Where you'll be working Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. What you'll be doing The Out of Home Care (OOHC) Health Pathways Program is a key initiative arising out of the Keep Them Safe: a Shared Approach to Child Wellbeing 2009-2014, which is the NSW Government Response to the Report of the Special Commission of Inquiry into Child Protection Services in NSW. The OOHC Liaison Nurse facilitates and implements health assessments and health management plans for children and young people entering Out of Home Care (OOHC) within Western Sydney Local Health District (WSLHD). The position will work within a multidisciplinary and multi-agency approach with key stakeholders both internal to Integrated & Community Heath Directorate and within WSLHD, and externally with key government and non-government partners including Department of Family & Community Services – Community Services, Department of Education & Communities, Western Sydney Primary Health Network (WentWest), General Practitioners, private health care providers, non-Government organisations, as well as children and young people in out of home care and their carer’s. The position has clinical, liaison, implementation and monitoring functions. Essential Criteria: Registration: Current Registered Nurse Division 1 with APHRA Selection Criteria: Minimum of three (3) years post-registration experience appropriate to performing the role of a Clinical Nurse Specialist Level 2 for children and young people in out of home care Demonstrated currency of practice and competence in assessing children and their families in relation to their health and social needs including an understanding of and commitment to Families NSW principles Demonstrated high level communication, liaison, networking and negotiation skills Demonstrated thorough understanding and ability to implement the principles of primary health care and work in partnership with families Demonstrated experience and knowledge working collaboratively as a member of a multidisciplinary team and with other service providers including Child and Family nurses, Allied health staff, General Practitioners, Child and Family Health Tertiary referral services Demonstrated ability to work independently whilst maintaining high level time management, problem solving, interpersonal and written and verbal communication skills.
