We encourage people of Aboriginal and Torres Strait Islander background and people with disability to apply.About the RoleAs a Care Coordinator you will oversee and enhance the delivery of our Aged Care services in Hervey Bay and surrounds. Some of your duties will involve processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit evolving needs. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.Additionally, you'll contribute to program coordination by assessing, improving, and overseeing operational and administrative systems. You will deliver exceptional service to ensure our clients and their families feel valued and supported, while also fostering the growth of our aged care services.This is a Permanent Full Time opportunity based in Hervey Bay.
