Care Coordinator

Join Life Without Barriers as a Care Coordinator and make a meaningful difference in the lives of elderly Australians.

We are seeking an experienced Care Coordinator to oversee and enhance the delivery of our Aged Care services in Rockhampton and surrounding local areas.

About the Role

The successful candidate will be responsible for processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit evolving needs.

You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.

Key Responsibilities

* Client Assessments: Undertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholders.
* Care Plan Monitoring: Ensure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visits.
* Risk Management: Monitoring the clinical risks and needs of a client, ensuring referral pathways are established for the safety of our clients.
* Collaboration: Collaborate effectively with internal stakeholders to ensure clients' needs are met including our Rostering, Practice and Workforce teams.
* Strategies and Services: Develop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities.
* Representation: Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community.

Skills and Experience

* Aged Care Experience: Demonstrated Aged Care or other clinical case management experience with strong administration skills, and ability to work with individuals, families and/or stakeholders.
* Industry Knowledge: Understanding of Home Care Packages, Commonwealth Home Support Program, Aged Care services and My Aged Care Standards.
* Clinical Expertise: Clinical experience and knowledge will be highly regarded.
* Technical Skills: High level competency in utilising a variety of technology including mobile devices and computers, proficiency with Microsoft Office, Outlook and database systems.
* Problem Solving: Ability to identify and implement risk mitigation strategies.
* Communication: High level communication, influencing, liaison and negotiation skills and established problem solving and conflict management skills.
* Autonomy: Ability to multi-task and work autonomously in a fast paced environment.
* Transportation: A current unrestricted Australian Driver Licence, reliable vehicle and a willingness to travel.
