About the Role:
This is an exciting opportunity for a dedicated healthcare professional to make a real difference in the lives of patients. As an Advanced Healthcare Practitioner, you will be responsible for conducting clinical assessments, reviewing cases, and providing recommendations.
You will work closely with ED MHT Clinical Nurse Consultants, Psychiatric Contact Liaison Service, ED and medical teams, Mental Health Line, Acute Care Team, and other community mental health teams.
Your Key Responsibilities:
* Conduct thorough clinical assessments to identify patients' needs and develop tailored care plans.
* Collaborate with multidisciplinary teams to ensure patients receive holistic care.
* Provide education and support to patients, families, and carers.
