About the role
The Nurse Home Visitor reports to and is directly accountable to the Director of Clinical Services and is responsible for the provision of the Australian Family Partnership Program (AFPP) along with the Pilbara Aboriginal Health Alliance (PAHA) Regional Nurse Coordinator as the lead partner agency for reporting and program fidelity.
Responsibilities
Using a caseload model, working alongside pregnant Aboriginal women and mothers to support them and their children (up to two years of age) through a comprehensive visiting service either in the home or in other community settings.
Working in partnership with maternal and child health staff, including local Aboriginal women employed as Family Support/Partnership Workers.
Providing culturally appropriate and clinically excellent care and support to pregnant women, their families and infants and young children, to promote healthy pregnancy and childhoods in alignment with the client centred principles including modelling therapeutic relationships; change theory; self-efficacy; client centred; strength based and solution focused approaches.
Supporting care coordination for Aboriginal pregnant women and mothers and their children.
Contributing to the evaluation and ongoing improvement of the AFPP and Upstream Health Project.
As per Section 30 of the Sex Discrimination Act **** (Cth) we seek to employ female Nurse Home Visitors for the effective and culturally appropriate delivery of services to the Martu and Nyiyaparli people.
About the practice
We are an Aboriginal Community Controlled Health Service located in the East Pilbara region of Western Australia and provide comprehensive and culturally appropriate primary health care services to the Martu people of Newman and the four remote Aboriginal communities of Jigalong, Parnngurr, Punmu and Kunawarritji. With an administrative hub based in Newman, in the Pilbara region of Western Australia, we provide holistic primary health care and community health programs services to the individuals and families within Newman and the remote communities of Jigalong, Parnngurr, Punmu and Kunawarritji.

Experience and qualifications required
Essential:
As per Section 30 of the Sex Discrimination Act **** (Cth) the occupant identifies as female.
Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse with a minimum 3 years' relevant clinical experience.
Demonstrated ability to communicate and empathise with First Nations people living in regional and remote communities.
Demonstrated competencies in and/or knowledge of the complexities and challenges involved in caring for Aboriginal children and their families.
Experience in providing support to pregnant Aboriginal women and/or mothers and young children that is culturally appropriate, trauma-informed and attachment-based.
Demonstrated ability to work effectively in a collaborative, multi-disciplinary environment.
Demonstrated ability to apply innovative solutions to complex problems in an unpredictable work environment.
Current (or willingness to obtain) immunisations training/certificate as appropriate.
Effective communication skills – written, verbal and interpersonal.
Demonstrated high level of professional integrity with regard to client privacy and confidentiality, ethics and creating a culturally safe environment.
Desirable:
Dual registration in midwifery and/or qualifications in child health and development.
The person is of Aboriginal and/or Torres Strait Islander descent and is acknowledged as such by their community.
Previous experience working within an Aboriginal Community Controlled Health service environment.
Experience using Communicare.

Additional information
This position offers the opportunity to work closely with remote Aboriginal communities and make a drastic impact to the health and well-being outcomes of these communities. We operate five sites, supported by a range of clinicians, community health workers and other ancillary workers. Although the successful candidates will have a substantive location (Newman) they must be prepared from time to time to work across any one of these sites should they be required.
