We are seeking a dedicated Team Leader to join our day services team, providing exceptional support to individuals with disabilities. In this role, you will be responsible for supervising and guiding a team of support workers, coordinating and delivering personalized activities, and ensuring a safe, inclusive, and engaging environment for all participants.
Key Responsibilities
* Participant Support & Engagement: Provide high-quality support to individuals with disabilities, empowering them to participate in activities that promote independence, personal growth, and social inclusion.
* Program Development & Delivery: Develop and lead a range of engaging day service activities tailored to the needs, goals, and interests of participants, including life skills, community access, recreation, and social engagement.
* Team Leadership: Mentor and supervise a team of support workers, providing guidance, training, and support to ensure high standards of care and professional development.
* Behavioral Support: Implement and monitor individualized behavior support plans, working closely with allied health professionals and families to ensure best practices are upheld.
* Documentation & Reporting: Accurately document daily observations, progress notes, incident reports, and any other required documentation, ensuring compliance with relevant policies and procedures.
* Health & Safety: Maintain a safe, supportive environment by conducting risk assessments, adhering to all health and safety protocols, and responding appropriately to any incidents.
* Stakeholder Collaboration: Work closely with families, allied health professionals, and other stakeholders to review and adapt support plans in line with participant goals and changing needs.
Requirements
* Experience: Minimum 3-5 years working in disability support or a related field, with experience in day services preferred.
* Qualifications: Certificate IV or Diploma in Disability, Community Services, or a related field.
* Leadership Skills: Proven ability to lead, mentor, and support a team, with strong interpersonal and communication skills.
* Person-Centered Focus: Demonstrated commitment to empowering individuals with disabilities through respectful, strengths-based support.
* Knowledge of NDIS: Familiarity with the National Disability Insurance Scheme (NDIS), including understanding of participant goals, behavior support plans, and reporting requirements.
* Behavioral Support Training: Certification or experience in positive behavior support (PBS) or similar approaches, with a proactive approach to managing behaviors of concern.
* Safety & Compliance: Knowledge of health and safety procedures, risk management, and safeguarding practices.
* Allied Health Knowledge: Experience working alongside or with allied health professionals to develop support plans.
* First Aid Certification: Current First Aid and CPR certification.
* Driver's License: Valid driver's license, as some travel may be required.
This is an ongoing schedule from Monday to Friday, 8:30am-4:30pm.
