We are seeking a compassionate and skilled Care Coordinator to join our team. The successful candidate will lead the delivery of high-quality clinical care for residents with complex needs.
This pivotal leadership role involves overseeing resident assessments, developing tailored care plans, coordinating clinical documentation, and ensuring compliance with relevant standards.
The ideal candidate will have experience in aged care or acute clinical settings, with strong leadership and workforce management skills. They must also possess excellent communication and interpersonal skills, as well as the ability to work collaboratively with external providers.
As a Care Coordinator, you will play a key role in promoting a culture of safety, wellbeing, and continuous improvement within our organisation.
You will be responsible for mentoring a team of Registered Nurses, care staff, and volunteers, and for monitoring and improving clinical programs and quality indicators.
To succeed in this role, you will need:
* AHPRA Registration as a Registered Nurse (RN) with a current Practising Certificate
* Minimum 3 years' post-graduate experience in aged care or acute clinical settings
* Proven leadership and workforce management skills
* Strong clinical assessment, care planning, and documentation capabilities
