In this role you will:
* Assist NDIS participants and their families/carers to identify needs through assessment and person-centred planning
* Evaluate, develop and deliver evidence-based Behaviour Support Plans in a complex support space
* Develop meaningful goals and practical strategies using a positive behaviour support framework
* Work with a commitment to eliminate restrictive practices in home and community settings
* Ensure plans reflect participants' goals, passions and strengths
* Build capacity and confidence of those who support participants, including family, carers and support networks
* Work collaboratively within a multidisciplinary team, including mentoring less experienced practitioners
About You
To be successful in this role you will have:
* A degree-level qualification as an allied health professional or similar (preferred)
* NDIS Commission suitability at Core level or above
* Report writing experience with high attention to detail
* Excellent time management and organisational skills to manage a varied caseload
* A proactive attitude and motivation to trial new ideas and contribute to improvement
* A genuine passion for working in the disability sector
* A current Working With Children Check & NDIS Worker Screening clearance
* Valid driver's licence and reliable vehicle
