Senior Care Lead
This is a leadership role within the clinical space, requiring a high level of clinical expertise and leadership skills. The successful candidate will be responsible for ensuring that all residents receive the highest quality of clinical care through effective management and coordination of assessment, care planning, and review processes.
Key Responsibilities:
* Lead clinical and care service teams across the care home to provide exceptional clinical care and support.
* Work closely with the Care Manager to ensure seamless coordination of carer teams, meeting all clinical needs of residents.
* Manage all aspects of assessment, documentation, and submission of AN-ACC requirements, maintaining accurate records in accordance with organisational and legislative requirements.
* Support the effective implementation of policies, procedures, and IT system requirements to enhance clinical care outcomes.
* Coordinate assessments, care planning, and review processes to ensure optimal care delivery.
* Support the admission process for new residents, providing comprehensive initial consultations and ongoing care assessments.
* Ensure timely and accurate recording of information in accordance with organisational and legislative requirements.
* Oversee facility team rosters and skill mix, ensuring optimal care delivery for residents.
* Identify and manage clinical risks, promoting a commitment to continuous improvement within the facility.
Requirements:
* A minimum of 12 months experience as a Clinical Care Coordinator or similar leadership position.
* Proven empathy and commitment to improving clinical care quality for residents.
* Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
* Postgraduate qualifications in aged care are advantageous but not essential.
* Extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC, and accreditation compliance.
* Strong influencing and leadership skills to develop clinical leadership capabilities across the clinical care team.
* High-level computing skills to maintain documentation and reporting requirements.
* Excellent communication, interpersonal, and teamwork skills, with a passion for leading clinical outcomes.
* Ability to manage multiple stakeholders and handle emergency situations effectively.
