Role Overview
The Aboriginal Youth Empowerment Program aims to provide culturally safe, trauma-informed, and individualized case management, mentoring, and goal setting for disengaged or at-risk Aboriginal youth.
The program empowers participants to achieve self-determination and positive life outcomes through strengths-based community mandates that focus on personal growth, emotional resilience, health, wellbeing, and employment outcomes with a strong emphasis on cultural identity, community connection, and sustainable impact.
* Implement intake referral processes in collaboration with the community
* Conduct initial and ongoing assessments of client needs, including risk assessments and case planning
* Provide proactive, persistent, reliable, practical, and comprehensive support that respects the dignity and privacy of individuals
* Contribute to identifying issues and setting goals and tasks that are mutually agreed upon and achievable for clients
* Facilitate multi-agency case management meetings and cooperate with other services as necessary and appropriate to support positive outcomes for clients
* Refer clients to external service providers to access specialist services
Required Skills and Qualifications
Candidates must possess a deep understanding of Aboriginal cultures and histories, as well as exceptional communication and interpersonal skills.
They should also have experience working with at-risk youth and a proven track record of delivering culturally safe and trauma-informed services.
