*The Role
An opportunity has become available for several Clinical Nurses (Chronic Disease and Post-Acute Programs) to join Gold Coast Health (GCH) where the focus of our strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
Provide case management and delivery of care at an advanced level
Liaise with multidisciplinary team ensure all aspects of recovery and self-management are considered
Facilitate a seamless transition across services to ensure continuity of care for clients
Assisting with communication and education relating to cardiac and chronic disease conditions
About you
To be successful in this position you will require:
Bachelor of Nursing or equivalent
Registration with the Nursing Midwifery Board of Australia
Driver's license
Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
Tuberculosis screening, to identify if the tuberculosis vaccination may be required
Declaration of serious discipline history must be disclosed
Criminal History Screening will be required for new employees (GCH will cover cost)

Salary
Permanent part time and temporary full time/ part time (one permanent part time position, 45.6 hrs p.f. and several temporary full time and temporary part time positions up to six months).
Base salary ranging from $ $ per fortnight, $ $60.05 per hour
About Us
Chronic Disease & Post-Acute programs (CD & PAP) provide community-based services for clients as a continuum of care for self-management, rehabilitation and secondary prevention. The Chronic diseases programs include Cardiac Rehab, Heart Failure, Respiratory, Diabetes, Falls & Balance, Cognitive Disorders, A&TSI Services, Chronic Kidney and Peripheral Vascular Disease clinics.
All programs aim to maximise medical therapy and develop the client's capacity to self-manage their chronic health condition.
The clinical nurse provides individual assessment, complex case coordination in conjunction with General Practitioner and Medical Decision Team (MDT), care provision, individual and group education, and group exercise sessions.
