About The Role

The NSW Health Aged Care Assessment Service (Hunter) is seeking a motivated and experienced Clinical Assessor to join our large and supportive multidisciplinary team. In this role, you will undertake comprehensive assessments of older people in both community and hospital settings to determine eligibility for Commonwealth funded aged care services and develop person-centred support plans. Clinical Assessor work closely with older people and their carers to understand their goals, preferences, and support needs. You will collaborate with Geriatricians, General Practitioners, Practice Nurses, other Health Services, and Non-Government organisations to ensure timely access to appropriate care and services. The role involves working across a variety of settings including public and private hospitals, residential aged care facilities (RACFs), and community environments, supporting older people to access the care they need.

Key Details

Hours: 8:00am - 4:30pm, Monday to Friday

Travel: Some travel required for client assessments and training

Training: Comprehensive orientation including the National Training Program (approximately 6 weeks, based at Wallsend)

Where You'll Be Working

The NSW Health Aged Care Assessment Service is a large, collaborative, and supportive team with the main office based in Wallsend. The team provides assessment services to older people across the community and hospital settings, supporting 26 public and private hospitals and 72 residential aged care facilities within these local government areas. The Service Covers the Hunter Region, Including Newcastle, Lake Macquarie, Port Stephens, Lower Hunter. Following the initial orientation and probation period, assessors will be advised of their primary work location, based on service demand and staffing needs across the region.

About You

You will bring the following:

* Previous experience working in aged care.
* Strong communication and interpersonal skills.
* Ability to work collaboratively with clients, families, carers, and service providers.
* Advanced assessment and problem‐solving skills.
* Excellent time management and organisational abilities.
* Ability to work both independently and within a multidisciplinary team.
* A high level of computer literacy.

Essential Requirements

Must hold a qualification in one of the following: Occupational Therapy, Nursing, Physiotherapy, Social Work, which provides eligibility or membership of the relevant professional association / provisional or general registration through the Australian Health Practitioner Regulation Agency (AHPRA). Eligibility to drive in NSW including willingness to travel to conduct home visit assessments.
