The Clinical Care Partner plays a pivotal role in supporting the delivery of Commonwealth funded aged care services in line with legislative, regulatory and contractual requirements.
In this rewarding role you will:
- Collaborate closely with operational teams to deliver clinical leadership for participants with complex care needs
- Conducting assessments, developing individualised care plans, maintaining effective communication with key stakeholders and identifying and responding to signs of clinical deterioration
- Combine hands-on clinical expertise with strategic oversight, ensuring that care planning, implementation and ongoing review are tailored to each participant’s unique goals, preferences and requirements.
- Provide recommendations to senior management on evidence based clinical matters that are aligned to best practice.
- Support a culture of open disclosure and continuous improvements through leadership and participation in activities that enhance overall operational capability

This is a permanent full-time role based out of our Gisborne, Bendigo, Maryborough, Echuca, Deniliquin, Finley, Corowa, Albury or Shepparton office.
This position has been classified under the Social, Community, Home Care and Disability Services Award as Level 5 with a starting annual salary of $100,776 + Superannuation + Salary Packaging.
What we’re looking for
We’re looking for someone with a Bachelor of Nursing or Bachelor (or higher) in Allied Health discipline (e.g. Physiotherapy, Occupational Therapy, Social Work etc) with five years’ experience in a similar role.
AHPRA Registration is also a requirement of the role.
